Joining any of our networks is easy. Which network—or networks—you join is entirely up to you. Whether you join Delta Dental Premier, our traditional fee-for-service program; Delta Dental PPO, our preferred provider organization; or DeltaCare, our managed care, capitation plan there are several items that you need to complete and sign:

• Participating Dentist's Agreement
• Participating Group Practice Agreement (only applicable if you have a group practice)
• Confidential Survey of Fees Form (for participation in the DeltaPremier Plan)
• Credentialing Application
• W-9 Form
