Who to Contact for Questions and Additional Information

For information on rates, plan designs or to order benefit materials:
Contact your Account Executive. If you do not know who your Account Executive is please call 1-800-451-1249 or (617) 886-1000 and ask for the Sales Department.

Ordering Supplies
To order:
· Enrollment forms and envelopes
· Monthly eligibility termination forms
· Claim forms
· Directories of Participating Dentists
· Supply order cards

Complete a Supply Order Card (PDF 12K) and fax it to our inventory supply firm at 1-800-477-9975 or drop it in the mail. Allow two to three weeks for delivery. Please contact your Account Executive if you do not have any supply order cards.

Billing
Contact our Billing Department at (617) 886-1025.

Member Services
Delta Dental Premier, Delta Dental PPO, Massachusetts Public Employees Fund and Group Insurance Commission members can receive automated information on:
· Enrollment
· Co-payments
· Deductibles
· Benefits

Call (617) 886-1234 or (800) 872-0500 and press 1 to be connected to the Voice Response Unit (VRU). Have the subscriber ID number (usually the social security number) and date of birth available. The VRU is not available during the following times:

M-F
· 3:30am - 4:15am
· 11:00pm - 11:45pm
Saturday
· 3:00pm - 3:30pm
Sunday
· 5:00am - 5:45am

Otherwise, the VRU is available 21 hours a day—seven days a week.
