How does our company
make changes to employee enrollment?
You may add, reinstate, terminate or make
status changes for employees or dependents that meet the Enrollment
Guidelines at any time during the year. All other enrollment
changes must be made during your company's open enrollment
period.

tried to download an Enrollment Form and received an error
message.
In order to view any of our forms on-line,
you will need to have Adobe Acrobat installed on your computer.
You may install Adobe Acrobat for free by clicking here
and following the instructions listed on Adobe's web site.
After you have completed your download, please return to the
Forms section of our web site and click on the Enrollment Form.
The form should now load and print for you.

Where
can my employees get information about their benefits?
We will send a supply of subscriber benefits
materials to your company for distribution to your employees
during enrollment and will replenish them upon request. Your
employees may also contact our Customer Service Voice Response

Some
of our employees speak a foreign language; do you have translation
services available?
Yes, we offer a foreign language translation
service through our Customer Service department. With the
help of the AT&T Language Line, Delta Dental of Massachusetts
communicates in 140 different languages. Simply have your employee(s)
contact our Customer Service
department to request a translator.

How can my employees
find a participating dentist near where they live or work?
Your employees may check Find
A Dentist on our web site. They will need to know if your
company's dental plan is Delta Dental Premier, Delta Dental PPO,
DeltaCare or the Value Plan program. They may check their
ID card for the name of their plan. Then, they will need to
enter in either their zip code or city (not both please).
A list of participating dentists will be displayed on the
screen that can also be printed out. Your employees may also
get a map with driving directions to the participating dentist
by clicking on their practice address.

What
are the benefits of receiving care from a participating dentist?
Participating dentists have agreed to accept
negotiated fees for services. When an employee receives care
from a participating dentist they enjoy:
- Credentialing standards.
All participating dentists meet the strict credentialing
standards of the National Committee for Quality Assurance (NCQA).
- No claims forms to fill out.
Participating dentists submit all patient claims, so there's
nothing for you or your employees to fill out.
- Lower out-of-pocket costs.
Participating dentists generally accept reduced fees and
your employees' co-payment (if applicable) is based on these
reduced fees. As a result, they pay lower out-of-pocket
costs than if they received care from a non-participating
dentist.
- No balance billing. Participating
dentists agree to accept Delta Dental's fee and the patient co-payment
or deductible (if applicable) as full payment.
- Direct payment. Delta Dental
pays the dentist directly, so your employees don't
have to pay the dentist the covered amount up front and
wait for a reimbursement check.

Can
my employees visit a non-participating dentist?
Yes, however services performed by out-of-network
dentists are covered at a lower benefit level and your employees'
out-of-pocket costs may be higher. Members receive the greatest
value when receiving care from a participating dentist.

Who
takes care of administering COBRA?
Delta Dental of Massachusetts complies
with COBRA rules and regulations and accepts existing COBRA
participants. We do not, however, assume the employer's responsibility
for notifying, billing and administering COBRA legislation.

does an employee replace a lost ID card?
They may click on the Contact Us section of our website and
complete the Customer Service form. Or they may contact
