- Guaranteed savings of at least 10% on all Massachusetts claims.
- Covers diagnostic and preventive care, including routine
checkups, and restorative care, including oral surgery.
- No claim forms to fill out when services are received
from a participating dentist.
- No balance billing by participating dentists.
- Covers services performed by non-participating dentists
at a lower benefit level.

- All participating dentists are credentialed to National
Committee for Quality Assurance (NCQA) standards.
- Provides claims savings of up to 25% over traditional
dental plans.
- Covers diagnostic and preventive care, including routine
checkups, and restorative care, including oral surgery.
- Participating dentists are reimbursed through a combination
of Delta Dental payments and patient co-payments.
- No claim forms to fill out when services are received
from a participating dentist.

The Delta Dental PPO Value Plan covers members 100% for diagnostic and preventive services only (no deductible) when you
receive care from a Delta Dental PPO dentist. Most basic and major restorative dental services are offered at significant
discounts when you receive care from a Delta Dental PPO dentist.

Voluntary Plans
These 100% employee-paid dental plans will
enable you to offer great dental coverage—at no cost
to your organization. We offer a choice of five voluntary plans, which offer great benefits at a flexible variety of price points. Regardless of which plan you choose,
your employees will:
- Be covered for preventive, diagnostic, minor restorative
and major restorative services.
- Never pay a deductible when they receive care from a network
dentist.
- Be eligible for $750 up to $1,000 in dental benefits annually.
- Have immediate access to dental care because there are
no waiting periods for coverage.
- Never be balance billed when they visit a participating
dentist.
- Never file a claim form when services are received from
a participating dentist.

National Coverage
Our membership in the nation's largest dental

to employees located outside Massachusetts. Delta Dental Premier,
